Longterm, expect social credit scores and passorts to require up to date vaccines and restrictions for entering any public venues, public transport and so on for violations. Likely tied to how you behave in public, online posts, race, income and other factors. China already has it to force public behaviours in a pseudo beta test for rest of world. Socialized medicine makes it very easy for the government to tie vaccines and other medical information to public access and requirements since you have no other options and they have the information already. I wouldnt expect it until UBI is implemented.
For immediate term, flights and inter city travel (longer buses, shuttles, etc) will likely require up to date vaccines to use them and some jobs might be able to get away with mandating it if they are government or deal with public (hospitals technically cant force vaccines on healthcare workers in case of flu that lost in court, but who knows what theyll be able to get away with). Children will likely be barred from attending school if they are not vaccinated. They will likely continue with the public shaming and making life miserable for those that do not comply, but the true restrictions will come in several years time.
In regards to preparing if you live in a city:
-pray. God's in charge and spiritual preparedness is way more important than physical preparedness.
-start growing food (even if its a few beans/easy to grow vegetables). Try to be at least partially self sufficient
-Research/learn/read about different survival skills/first aid (better to have the skills/knowledge and not need it than to need it and not have it). And who knows what will happen.
-keep a low profile. Stay informed, but remember that most people won't be willing to change their minds, even if you have solid proof. Praying for them probably does more good than arguing. God can change even the hardest hearts...we can't do that.
I plan on fleeing far before that. If I have to cross the border illegally then so be it. But I'm sure as shit not staying here long enough to give my "Government" A chance to lock me in.
Not sure how much better the US will be. There's a bigger Christian community and more pushback, but overall still corrupt and prone to technocratic powergrabs.
Not sure where would be best, but if it's going to be a technocracy, some place low tech.
I don't have solid recommendations - India? South america? Africa? Former soviet countries? I don't know.

You will own nothing and you will love it.
