In both USA and Canada not even private businesses selling goods and services can deny service based on a protected group, ie race, religion, sex, sexuality, disabilities, etc.
Mask medical exemption falls under the category of discrimination against those with disabilities. In Canada you do not need to provide proof, I'm unsure of the USA.
If a private store has a policy that might impact on certain protected groups they have to at least provide reasonable accommodation; in some cases reasonable accommodation is still too discriminatory so normal accommodation must be provided.
However in the case of individuals not claiming medical exemption and who are instead rejecting store policy without the protection of discrimination laws, then that would probably be a case where a private business's policy is not regulated by license and the business can reserve their right to refuse service in that case.
