Go to file --> make a copy. Then change your name and date of birth to match your ID.

If there's enough demand for this, there's a way to make this fake ID better because it looks slightly different compared to the PDF. (also, there are ways to auto-generate different doctor/nurse/pharmacists names and vaccination sites)
